Do It Online

During the current COVID-19 situation you are able to register for online access by entering your details into the comments and suggestions form (click here). Once submitted a member of our team will be in touch with further instructions.

Please note: 

  • It is with regret that we inform you it is not currently possible to book practice nurse or health care assistant appointments online.
  • Proxy Access to Online Services for children is automatically withdrawn at the age of 14. At the age of 16 patients are able to have their own access to Online Services. Proxy access can be requested for a child after the age of 14 by completing and submitting a new request. The child will be invited to attend an appointment with a clinician where the implications of this will be discussed and competency assessed. The clinician will decide the appropriateness of the access request and, if approving proxy access, if any further review is required prior to the age of 16.

Subject Access Requests

If you would like to request information from your medical record or for a child you have responsibility that is not available/viewable on the online record then please complete the Subject Access Request form and return to the surgery via post or in person FAO the Practice Manager. There will usually be no charge for this. To download a copy of the form, click here.

Completion of forms, reports or letters from GP

If you would like your GP to complete a form or report or write a letter for yourself or for a child you have responsibility for please complete the Patient Request Form and bring it in person to Reception. There will usually be a charge for this and you will be asked for a £20.00 deposit. You can get a request form from reception or to download a copy of the form click here.
